These are the forms you will need to fill out for your visit. Please print these out and read them carefully. They include:
- - information about the few people who should not have this treatment
- - instructions for you to follow before your appointment
- - the usual intake and consent forms
Fill them out and bring to your first appointment. Alternatively, if you are unable to print these out, you can fill them out when you arrive. Please let me know so I will have them waiting for you and come in about 15 minutes early to fill them out.
